Being confined to a wheel chair and relying on family, friends and caregivers to
help move can be a life‑long struggle for an individual with a physical disability.
All Abilities recently helped coordinate the construction of a ramp for a client with
Cerebral Palsy. The new ramp gave the client the ability to enter and exit the home independent of help from others. The client and family were grateful for the services provided and for their loved one’s newly found freedom.
The Commonwealth of Pennsylvania has revised the implementation timeline for the Community HealthChoices roll-out. Phase 1 in the Southwest region (counties shown on the map) began on January 1, 2018 as previously planned. Regularly scheduled services will not be interrupted.
The implementation timeline for Phases 2 has been changed to begin on January 1, 2019 for the Southeast region (affected counties shown on the map) and Phase 3 implementation has been changed to begin on January 1, 2020 for the rest of the state counties (shown on the map).
Community HealthChoices (CHC) is a new managed care initiative started January 1, 2018. CHC will oversee physical health care and long-term services and supports (LTSS) for persons with physical disabilities, older persons, and those who are dually eligible for Medicare and Medicaid. These services will be coordinated by a Managed Care Organization (MCO) instead of the Commonwealth of Pennsylvania.
